PLEASE WATCH FOR UPCOMING INFORMATION VIA EMAILS TO FAMILIES REGARDING A NEW PROCESS FOR ONLINE REGISTRATION FOR SPORTS, CLUBS AND PARKING VIA INFINITE CAMPUS.  FOR THE UPCOMING 2019-20 SCHOOL YEAR, WE WILL NOT BE USING THE PROCESS THROUGH THIS SITE BUT RATHER THROUGH INFINITE CAMPUS.  PLEASE WATCH FOR FUTURE EMAILS.

PHYSICAL EXAMINATION FORMS:  

• Students registering for athletics and strength and conditioning club MUST have a physical examination on file *before the student is authorized to participate in practice/tryouts*. To ensure clearance for participation, physicals should be completed no later than two business days prior to the first day of practice/tryouts.
• HIGH SCHOOL - A physical exam is valid for the 2019-20 school year if it took place on or after April 1, 2018.  The physical exam date, medical provider's signature, and the physician's name, address and phone number are required to be on the form.
• MIDDLE SCHOOL - A physical exam is valid for four years if it took place after April 1st of fourth grade.
• If the physical exam took place prior to April 1, 2018, the high school student will need a new physical for the 2019-20 school year.
